1: Attending your surgery
You have recently attended an appointment and had your blood
pressure checked. You have then been asked if you would like to
monitor your own BP @ Home for the next 7 days with BP Monitor
provided by your surgery (Unless you already have your own)
2: Completing the EMIS template
If you have agreed to the above - Your
doctor/nurse will then complete the requested
BP@Home referral form.
3: Once the form has been completed
Once the doctor/nurse has inputted your details onto
the referral form it will then to be emailed to:
remote.monitoringchoc@nhs.net
4: Cumbria Health Contact
Once the referral has been sent to Cumbria Health, you will be
contacted to get you set up on the BP@Home Service.
Their are several ways we can collect your readings.
- A daily phone call
- A text message twice a day
- An email twice a day
5: Advice
If a reading is abnormally high, you will be asked to repeat your
blood pressure reading up to three times. After the third
reading you will get another message informing you to ring
your own GP or Out of hours service within the next 24hrs if it
continues to be abnormally high.
6: End of service
After 7 days of monitoring your blood pressure a PDF Document
will be produced and sent to your GP practice, this will include all
of your readings that you have submitted.
If you have borrowed a BP Monitor, this must be returned to your
GP Practice at your earliest convenience.
